How Do I Apply?


Greetings from the Family Partnership Staff

Community Action Partnership Head Start and Early Head Start serves expectant families, families with children ages 0-3 and families with children ages 3-5 who meet eligibility requirements based on age and income.

How to Apply for Early Head Start or Head Start

If you are interested in one of our program options, please contact a Family Partnership case manager at 701-227-3010 or toll free at 1-877-546-9420 to make an intake appointment.

At the appointment, we will ask for the following:

  • Proof of your family’s income
    • 2023 w2’s or 1040 tax form
    • Unemployment Notification letter
    • Social Security (SSI) award letter
    • SNAP benefit letter
    • TANF benefit letter
    • Pension or annuity payments
    • Military income
  • Your child’s immunization record
  • Your child’s birth certificate.
  • Any legal custody paperwork.

Please complete the intake form and application form prior to your intake appointment.  You may stop by our office to pick up a packet or have one mailed to you.
